Quality Assurance
At Sioux Lookout First Nations Health Authority (SLFNHA), Quality Assurance (QA) is how we ensure our services are safe, effective, and centered on the needs of clients, families, and communities. QA is not just about compliance—it is about learning from experience, strengthening how we work, and making improvements that support long-term wellness.
Through QA, we support health transformation by ensuring programs and services evolve in ways that reflect community priorities, advance cultural safety, and build stronger systems of care. QA also enhances the service experience by focusing on the quality of every interaction, making sure clients feel heard, respected, and supported throughout their care journey.
When we review incidents, improve processes, or measure outcomes, we do so in ways that honour respect, relationships, culture, equity, and fairness. QA is both a reflection of our values and a way of holding ourselves accountable to them—ensuring that every improvement we make strengthens trust and supports resilient, healthy Nations on their path to wellness.
Our QA Work Includes:
- Compliments & Complaints Reporting – Ensuring community voices guide improvements.
- Incident & Safety Reporting – Learning from events to keep clients and staff safe.
- Evaluation & Accreditation – Measuring ourselves against national standards.
- Continuous Improvement – Identifying what works well and where we can do better.
Compliments & Complaints Reporting
Your feedback is essential. Rooted in respect. Guided by Stories.
Your Story Matters
- Compliments recognize staff and programs that go above and beyond.
- Complaints identify concerns that need to be addressed to improve fairness, safety, and client experience.
We Honour Your Story
At SLFNHA, we believe that every story carries meaning. When you share your experiences—good or bad—you help us walk a better path together. Our Compliments & Complaints Program is a safe and respectful space where your story can be heard, held, and used to make care better for everyone.
Speak in the Language of your Heart
We understand that stories are best shared in your own language. Whether you want to speak Anishinaabemowin, (Ojibway, Oji-Cree, Cree) or English, we have someone who is there to listen and understand.
Why Share Your Story?
Your Stories help us:
- Celebrate care that was kind, safe, and healing.
- Understand where things may have gone wrong.
- Make changes that reflect the values of our people.
- Strengthen trust and relationships across the system.
What Happens with Your Story?
- We listen with respect.
- We take your story seriously.
- We look at what needs to be changed or celebrated.
- We follow up—if you want us to.
