Sioux Lookout Area Primary Care Team
SLFNHA has been successful in securing funding to support a mobile Indigenous Interdisciplinary Primary Care Team. Implementation of this program is in the beginning stages and SLFNHA will be working with key partners, including communities, funders and other service providing organizations to further develop the service delivery model to ensure it meets community needs. Funded by the Ministry of Health and Long Term Care (MOHLTC), this team will provide services to the SLFNHA communities as well as the Municipality of Sioux Lookout. We are actively recruiting for many positions and are beginning to provide services.
The SLAPCT will operate in a team environment, to provide wrap-around services and seamless primary care. The model will involve smaller teams responsible for a cluster of communities which will travel to those communities on a regular basis to provide collaborative care.
- Nursing (Registered Nurses, Registered Practical Nurses, Nurse Practitioners)
- Speech Therapy
- Occupational Therapy
- Physiotherapy & Kinesiology
- Registered Dietician
- Mental Health (Social Work and Psychology)
- Community Health Workers
- Traditional Healing
In addition, SLFNHA has also received a capital planning grant to support the capital needs associated with providing the above health care services in Sioux Lookout and in First Nation Communities. To support this process, SLFNHA has identified a Capital Steering Committee and the inaugural meeting will be held in Sioux Lookout on February 21st and 22nd, 2018. The Steering Committee members will be leading the process of development of a business plan and capital plan, that will assist in identifying the primary care needs of each community, which may include various approaches such as new builds or retrofits of existing buildings.
Ongoing updates will be provided as this exciting new Primary Care Team evolves and crucial timelines are met regarding the Capital project.
