JOB SUMMARY:

Under the Sioux Lookout Area Primary Care Team (SLAPCT) and within the scope of practice of a Social Worker, the Social Worker will be responsible for providing primary health care to clients within the Sioux Lookout area. The Social Worker will work closely with clients and communities enrolled in the Hep C Program. The Social Worker promotes healthy lifestyles and disease prevention, as well as collaborates and consults with members of an interdisciplinary team to address issues experienced by clients and within the community.  The Social Worker will provide holistic and integrated care to all ages within the Sioux Lookout catchment area, including the First Nations communities where the Hep C program has been successfully integrated.

The Social Worker is directly accountable to the Primary Care Team Pod Manager.

QUALIFICATIONS:

  • Persons of First Nations ancestry will be given preference (OHRC, Part II, Special Employment)
  • Registration in good standing with The Ontario College of Social Workers and Social Service Workers.
  • New graduates are welcome to apply, however preference will be given to those with experience working as a Social Worker in a clinical and community setting.
  • Experience in the provision of Mental Health counselling services to First Nations people and/or their families.
  • Knowledge of the First Nations culture and the principle issues affecting the mental health of First Nations clients, families and communities in the Sioux Lookout catchment area.
  • Experience with various psychotherapies, such as cognitive behavioural therapy, dialectical behavioral therapy, motivational counselling, group therapy, family therapy and solution focused therapy.
  • Demonstrates ability to work effectively in a multi-disciplinary environment.
  • Ability to provide coaching, mentorship, and guidance to community-based health workers.
  • Excellent relationship-building skills to initiate and maintain working relationships with management, and internal and external stakeholders.
  • Excellent verbal and written communication skills.
  • Experience with electronic medical records, health information databases and strong computer skills.
  • The ability to function effectively during periods of rapid change and transition.
  • Knowledge of or willingness to learn about communicable diseases specific to the population we serve.
  • Ability to communicate in one of the First Nations dialects of the Sioux Lookout region is an asset.
  • Must be willing to travel by various modes
  • Must have a valid Driver’s License
  • Must be willing to relocate and live in Sioux Lookout (or within a daily commuting distance)
  • The ability to perform the requirements of the position on a regular basis.

ROLES AND RESPONSIBILITIES:

  • Knowledge to support clients as they learn to manage and navigate diagnosis’ of Chronic Illness, specifically communicable diseases such as Hepatitis C.
  • Coordinate and provision of clinical assessments/ therapy/ counselling/ support/ treatment plans/ interventions and follow-up, utilising clinical reasoning.
  • Assess, treat and monitor clients specific to their healthcare needs and overall health, specifically their mental health and wellbeing.
  • Educate and counsel clients and their caregivers on strategies and tips on how to live a healthy lifestyle and maintain an overall state of wellbeing.
  • Work with other health providers and specialists to formulate prognosis and recommend best course of intervention.
  • Work with clients and communities to promote healthy lifestyles, disease prevention and community health.
  • Performs administrative duties in support of the organization’s goals and objectives.
  • Perform other duties as assigned within the scope of the position.
