HCV Case Coordinator

As part of the Approaches to Community Wellbeing’s Preventing Infectious Diseases team, the HCV Case Coordinator will work with the Hepatitis C Support and Treatment Service to coordinate clinics, and act as a resource to all First Nations people who reside in the 33 communities served by SLFNHA who are at risk of acquiring, living with, or affected by the hepatitis C virus (HCV).
The HCV Case Coordinator is a combined role offering case management support to clients as well as administrative support to the HCV Treatment team. The position supports, guides, educates, and helps to coordinate comprehensive care and treatment to ensure those at risk of acquiring, living with, or affected by HCV are supported throughout their care.

Qualifications:

 

  • Diploma or degree in a health-related field
  • Minimum of 2-years of experience related to health services delivery and case management
  • Knowledgeable about Hepatitis C prevention and transmission, harm reduction approaches, treatment options and regional services involved in HCV care
  • Experience providing support to clients with health issues and/or chronic diseases, and helping clients develop sources of support
  • Experience coordinating a multidisciplinary clinic is an asset
  • Must have excellent written and verbal communication skills
  • Ability to develop and maintain respectful, cooperative working relationships with First Nations people, communities, service workers and health care providers to promote an integrated seamless delivery of service
  • Knowledgeable of First Nations culture and principle issues affecting mental health and substance abuse among First Nations people
  • Experience working with First Nations health programs
  • Ability to communicate in a First Nations language is an asset
  • Professional integrity and the ability to maintain confidentiality is essential;
  • Familiarity with Microsoft Office software
  • Ability to gather and maintain accurate documentation and provide reports as required in a timely manner
  • Ability to schedule meetings, produce  minutes and organize travel
  • Travel required as necessary

 

 

 

 

Responsibilities:

 

Administrative Responsibilities

  • Plan and co-ordinate multidisciplinary HCV clinic, both at the Primary Health and in northern communities as required by arranging travel for the team, organize space for clinics, complete paper work for clients, and notify clients of their appointments
  • Schedule meetings, prepare agendas and record minutes
  • Receive and answer requests for information, made by telephone or in person, from the general public and other community agencies.  Convey messages to team members, other departments, and outside agencies
  • Assemble materials for education and/or administrative purposes and distribute as required
  • Order and maintain office supplies and educational resources for the HCV team

 

Case Coordination Responsibilities

  • Accept referrals into the HCV Program
  • Connect with clients referred into the HCV Program to explain the treatment process and to identify the clients’ educational needs
  • Provide case management support to clients
    • Identify client educational needs and link clients to educational resources, or other educational programs if appropriate
    • Conduct one-on-one education on HCV and harm reduction with clients in the program
    • Connect client with the Treatment Nurse to discuss treatment options, side effects, complications, etc. to support retention in treatment or therapy
    • Refer, or facilitate the referral process of, clients for counselling or other health services, as appropriate
    • Support clients in referrals related to the social determinants of health (i.e. housing, income support, etc.)
    • Coordinate (time, location, agenda & minutes) as well as participate in HCV team meetings to provide pertinent
    • Provide information to the Treatment Nurse and Treating Physicians
    • Maintain confidentiality of client interactions and information
    • Document client interactions and information to identify, track, and monitor those being supported for effective surveillance and continuity of care and store in accordance with SLFNHA privacy policies regarding personal health information
  • Coordinate support and collaborate with the system responding to HCV to reduce gaps in service for seamless health care delivery
  • Perform any other duties assigned by Manager
